MAJOR DUTIES & RESPONSIBILITIES |
| Case worker will provide direct client services including, but not limited to: emergency and crisis – focused case management, rapport building, client intake and eligibility determination, evaluate data for individual client progress, develop individualized service planning in response to assessments, identify and coordinate appropriate services (inside and outside of the agency), provide client advocacy, complete case monitoring, and provide routine aftercare services. |
| Case worker will coordinate with other agencies to ensure appropriate services are obtained for clients. The case worker will be a liaison between client and other agencies. The caseworker will staff cases with director, and professional service providers. |
| Case worker will collaborate with social services agencies that assist victims of crimes such as Houston Area Women’s Center, local shelters, legal aid service providers, HHSC and others as part of a holistic approach to service provision. |
| Monitor clients’ progress to ensure that their goals and objectives are met,?particularly those related to VOCA program requirements. |
| Reports to Director as required by state standards, and is responsible for caseworker planning, intake assessments, admissions, permanency plans and record keeping of clients/families of TCCC and families. Write case notes and maintain client files and documentation. Complete required data entry to ensure client files are audit compliant. |
| Performs other duties as assigned. |
