    Position Summary

     

    The Transition Center & Outplacement Coordinator oversees the District’s Transition Center, ensuring high-quality programming and services for post-secondary students with disabilities. This role focuses on supporting students’ transition to employment, independent living, and continued education/training while ensuring compliance with federal, state, and local regulations.

     


    Minimum Qualifications

     

    Education & Experience

     
    • Master’s degree in Special Education or a related field.

    • Minimum of four (4) years of experience working with transition-aged students with disabilities in an educational or vocational setting.

     

    Licensure & Certification

     
    • Illinois Professional Educator License with required endorsements per the Illinois School Code.

     

    Knowledge, Skills & Abilities

     
    • Strong understanding of transition planning, vocational education, adult services, and community-based instruction.

    • Knowledge of IDEA transition requirements and legal compliance.

    • Ability to develop and implement individualized transition plans aligned with student IEPs.

    • Skilled in collaboration with families, adult agencies, community partners, and employers.

    • Experience in managing budgets, coordinating transportation, and overseeing job sites.

    • Strong leadership, organizational, and communication skills.

    • Familiarity with postsecondary outcome tracking and data systems.

     


    Essential Duties & Responsibilities

     
    • Oversee daily operations of the Transition Center, including staffing, program development, and logistics.

    • Supervise and evaluate Transition Center staff to ensure effective instruction and student support.

    • Develop partnerships with adult service agencies, businesses, vocational programs, and postsecondary institutions.

    • Coordinate community-based instruction, job training, internships, and volunteer opportunities.

    • Ensure compliance with all IDEA transition mandates and state regulations.

    • Collaborate with families and IEP teams to establish postsecondary goals and meaningful transition outcomes.

    • Plan and facilitate transition-related IEP meetings and student-centered planning sessions.

    • Maintain accurate student records, including progress reports and employment experience tracking.

    • Provide instruction in independent living skills, employability training, travel training, and self-advocacy.

    • Monitor and assess student performance in job placements and community settings.

    • Coordinate transportation services for community-based activities.

    • Design and implement transition programming curricula aligned with best practices in adult special education.

    • Communicate regularly with district administration regarding program needs, goals, and student outcomes.

    • Prepare reports and data for compliance monitoring and program evaluation.

    • Facilitate professional development for staff related to transition services and adult learning strategies.

    • Advocate for appropriate postsecondary supports and services for students.

    • Ensure adherence to safety protocols across all program settings.

    • Manage the Transition Center budget and allocate resources effectively.

    • Perform other related duties as assigned.

     


    Supervisory Responsibilities

     
    • Direct supervision of Transition Center staff, including job coaches, program assistants, and certified educators.
