8. Prepare reports as requested by the supervisor/administrator
9. Work on special projects and assignments as requested
10. Perform data processing duties, such as: entering and updating scheduling information, making changes to student records as necessary and processing ongoing information for state and federal reports
11. Work cooperatively with appropriate personnel to monitor and collect supportive documentation for local, state and federally funded programs
12. Identify, analyze and solve problems
13. Perform other duties as assigned by the superintendent or his designee.

SCOPE OF POSITION:    To coordinate transition planning and services for students with disabilities, ages 14-21, in accordance with the current NJDOE regulations.  Services should include, but not be limited to, planning for post-secondary schooling, employment, and independent living through the design and implementation of appropriate programs, both within the school and in the wider community, liaison with community agencies and government entities for access to services as well as transition to post-secondary services, and coordinated oversight of pupil progress once transition activities have commenced.  The services themselves may be provided by district staff or by contracted providers.

DUTIES AND RESPONSIBILITIES:   
• Liaise with CST, Guidance Counselors, Special Education Teachers, school administrators, and Director of Pupil Services in reference to individual students and Transition issues
• Participate in development of transition plans for all students requiring services beyond those available from CST and Guidance Counselor
• Provide oversight of transition plans for all students requiring services beyond those available from CST/Guidance Counselor
• Plan, develop, and participate in implementation of school-to-work program at Morristown High School, under the direction of Director of Pupil Services and school administration
• Develop community resources in support of school-to-work program
• Develop relationships with community and governmental agencies (private providers, DDD, DVR, ARC, etc.)
• Conduct outreach activities to ensure successful participation by students enrolled in community-based work activities
• Collaborate with Curriculum Department and faculty to improve curricular response to student learning needs
• Communicate with parents in regard to individual students
• Provide parent education opportunities related to transition
• Perform other related duties as required by supervisor or as needed to ensure student welfare
CONTRACT TERMS:     10-months
