    General Function:
    The Adelante Early Childhood Case Manager works to improve services for families enrolled in the Adelante program with young children ages 5 and under. The case manager is responsible for the coordinatation of support and services when there are needs expressed by Santa Fe Public Schools families experiencing homelessness. The case manager is responsible for assessing needs, developing a plan of support,coordinating services, and monitoring the outcome for the families and young children who are served.
     
     
    Education/Training/Experience:

    Minimum:   Bachelor’s Degree in Social Work or other human services degree from an accredited university

             2 Years of early childhood experience
             Must be bilingual in Spanish and English
             12 months experience working with families living in poverty
             1 Year case management experience  

    Preferred:   Bilingual: English-Spanish.

    Master’s Degree in Social work or other human services degree from an accredited university
    Experience working in a school system
    Infant Mental Health Certification

     
     
    Technology Skills

    Proficient in Google Suite (Email, Calendar, Sheets, Forms, Docs, Meets)
    Skilled at engaging with families via multiple social media platforms 
    Ability to use various on-line platforms for data entry

    Understanding homelessness and the needs of students and families experiencing homelessness
    Understanding of local community services available to families living in homelessness and extreme poverty
    Knowledge of early childhood educational and 
    Awareness of best practices relating to working with adults in crisis

    Ability to create a calm, safe environment in high intensity situations
    Ability to assist families in budget planning and goal setting

    Salary Range
    $42,00-$52,000+ (Depends on experience and level of degree)

    Ability to lift up to 50 pounds frequently( mostly food)
    Manual dexterity to operate office equipment

     
    Environmental Conditions:
    Regularly required to stand and move throughout a room and/or facility. Communicates verbally and electronically. Uses hands to handle or feel objects, tools, or controls and reach with hands and arms. Frequently required to stoop, kneel, crouch, and/or crawl. Required to sit, climb, and balance. Heavy lifting up to 50 pounds. Specific vision abilities required include close vision, distance vision, color vision, and the ability to adjust focus. Must possess and maintain a valid driver’s license and private automobile. Travel may be required.  

    Essential Job Functions:
    All functions are performed in accordance with established policies, procedures, safety and environmental regulations, facility and computer security policies and procedures. Functions are subject to change from time to time to meet the needs of the District. Current essential functions include but are not limited to the following:

    Conduct intakes and screenings to assess needs of parents/guardians that are living in high risk/homeless situations as identified under McKinney-Vento Homeless Assistance Act and whose child(ren)is/are 4 years of age or younger.
    Develop family plans  with the goal of self-sufficiency
    Make referrals and linkages for families in need of housing, medical care, mental health support and substance abuse treatment, food, other necessities and social services in a timely manner
    Work collaboratively with Santa Fe Public Schools Early Children Program Coordinator and local agencies  to support enrollment in early childhood programs
    Track and document referrals and linkages through an on-line platform
    Facilitate meetings with local early head start programs and parents regarding possible services
    Track the number of families and children served, services provided and outcomes
    On an as-needed basis, make home visits and assist adult family members with appointments
    Represent SFPS Adelante at agency meetings/public events as necessary 
    Research and inform SFPS Adelante staff/community members of the  effects of child/family homelessness, best practices in the field and reducing secondary trauma in staff members. 
    Continue to seek and engage new community resources on behalf of Adelante children
    Participate in weekly staff meetings facilitated by the Adelente Program Coordinator.
    Engage in meaningful conversation about the work, including, but not limited to, challenges and success.
    Meet with the program coordinator to obtain feedback and guidance on work performed  
    Participate in fundraising events and evening events as scheduled by the coordinator. 
    Provide assistance in packing school supplies, food and/or other program needs as necessary. 
    Provide data and complete required paperwork in a timely manner
    Collaborate with community/school personnel as necessary to help stabilize Adelante families so that students reach their highest potential
    Work collaboratively with outside agencies for the betterment of students and families experiencing home and food insecurity
    Participate in weekly food and item distribution, specific to early childhood items/needs
    Other duties as assigned.

    Protects the District’s resources through appropriate and careful use of supplies and equipment.
    Utilizes appropriate body mechanics to aid in the prevention of muscle strain/injury.
