Responsibilities
 
• POSITION PURPOSE: This position is responsible for providing case management support for Special Education Teacher caseloads

• This includes collaborating with Special Education Teacher for student assessment, drafting student IEPs, meeting coordination, attendance, facilitation, paperwork completion and processing, and records management (audit files and student management computer system)

• The Special Education Case Manager will work closely with the instructional personnel for students with disabilities including speech and language pathologists, occupational and physical therapists, school psychologists, school counselors, and social workers to determine eligibility, placement, and programming for special education services

• Compile, maintain, and electronically file all reports, records, and meeting paperwork, and submit required components to district office according to established timelines

• Develop, monitor, and report on goals; determine needs, design interventions utilizing research-based best practices

• Provide clear documentation in designated platforms that support the progress and growth of students toward their documented goals

• Participate in IEP meetings and other meetings as LEA

• Collaborate with the Special Education Teacher(s) to select assessments and materials, establish system for data collection, and collection and organize all completed relevant assessment data

• Utilize data collected from school psychologists, special education teachers and therapists, to draft student’s initial, annual, and amendment IEPs in collaboration with the student’s team lead and within the district’ required preparation timelines

• Performs other related tasks as assigned by the building principal.

• Other supervisory duties as required by the building principal.

• This includes collaborating with Special Education Teacher for student assessment, drafting student IEPs, meeting coordination, attendance, facilitation, paperwork completion and processing, and records management (audit files and student management computer system)

• The Special Education Case Manager will work closely with the instructional personnel for students with disabilities including speech and language pathologists, occupational and physical therapists, school psychologists, school counselors, and social workers to determine eligibility, placement, and programming for special education services

• Compile, maintain, and electronically file all reports, records, and meeting paperwork, and submit required components to district office according to established timelines

• Develop, monitor, and report on goals; determine needs, design interventions utilizing research-based best practices

• Provide clear documentation in designated platforms that support the progress and growth of students toward their documented goals

• Participate in IEP meetings and other meetings as LEA

• Collaborate with the Special Education Teacher(s) to select assessments and materials, establish system for data collection, and collection and organize all completed relevant assessment data

• Utilize data collected from school psychologists, special education teachers and therapists, to draft student’s initial, annual, and amendment IEPs in collaboration with the student’s team lead and within the district’ required preparation timelines

• Performs other related tasks as assigned by the building principal.

• Other supervisory duties as required by the building principal.

• POSITION PURPOSE: This position is responsible for providing case management support for Special Education Teacher caseloads

• This includes collaborating with Special Education Teacher for student assessment, drafting student IEPs, meeting coordination, attendance, facilitation, paperwork completion and processing, and records management (audit files and student management computer system)

• The Special Education Case Manager will work closely with the instructional personnel for students with disabilities including speech and language pathologists, occupational and physical therapists, school psychologists, school counselors, and social workers to determine eligibility, placement, and programming for special education services

• Compile, maintain, and electronically file all reports, records, and meeting paperwork, and submit required components to district office according to established timelines

• Develop, monitor, and report on goals; determine needs, design interventions utilizing research-based best practices

• Provide clear documentation in designated platforms that support the progress and growth of students toward their documented goals

• Participate in IEP meetings and other meetings as LEA

• Collaborate with the Special Education Teacher(s) to select assessments and materials, establish system for data collection, and collection and organize all completed relevant assessment data

REPORTS TO: Executive Director of Student Support Services

GENERAL RESPONSIBILITIES: Physical therapy assistants provide services under the supervision and direction of a licensed physical therapist for special education students. These services provide essential support to individual education programs as determined by the IEP team. PTAs implement selected components of interventions/treatment, obtain data related to the interventions provided, and make modifications in selected interventions either to progress the student as directed by the physical therapist or to ensure student safety and comfort.
 
Performance Responsibilities
1. Collaborate to develop individual goals and objectives for students based upon previous and current progress, areas of deficits and needs to be incorporated into the student’s IEP.
2. Under the supervision of a physical therapist, implement appropriate physical therapy services to support the goals and objectives of a student’s IEP.
3. Identify environmental needs/barriers and make recommendations for accommodations/physical modifications under the supervision of a physical therapist.
4. Independently conducts both individual and group physical therapy treatments as assigned by the physical therapist
5. Monitors student’s response to intervention and consults with the physical therapist if modification is indicated.
6. Assists in the selection of physical therapy techniques/media and in the sequence of activities, adapts techniques/media as indicated by student performance and following consultation with the physical therapist.
7. Records billable treatment times and provides documentation for students receiving MEDICAID according to school system guidelines.
8. Participate in staff development activities for personnel.
9. Attend staff meetings and other school meetings when appropriate.
10. Maintain and clean supplies, equipment and other therapy areas.
11. Maintain required documentation and data collection of therapy sessions.
12. Follow the division’s policies and procedures as related to guidelines and instructional initiatives.
13. Consistently demonstrates sound judgment in the implementation, planning, and follow-up of student therapy programs.
14. Perform other relevant duties and responsibilities as assigned by the supervisor.
 
PHYSICAL REQUIREMENTS
Must have the use of sensory skills in order to effectively communicate and interact with others through the use of the telephone and personal contact as normally defined by the ability to see, read, talk, hear, handle or feel objects and controls. Standing, walking, siting, stooping, kneeling, bending, and crouching are regularly required of the position. The employee is required to have visual and hearing acuity sufficient enough to assess patient safety and ability.
