PURPOSE OF THE POSITION: To provide child development and family support services to families parenting children prenatal through age 3 to help them prepare their young children for school success.
SUPERVISORY RESPONSIBILITIES: None

- Ensures that all actions are in concert with the Board policies as well as the mission statement, beliefs, objectives, and parameters found in the Board’s strategic plan.

- Performs other related duties as assigned for the purpose of ensuring an efficient and effective work environment.
- Provides culturally sensitive, intensive, and comprehensive evidence-based child development and family support services
- Maintains active caseload at capacity based on Home Visiting Model guidelines and funder requirements.
- Recruits, screens, and identifies eligible children and families for the Home Visiting Program.
- During the child find process, refers children suspected of having a delay to Child and Family Connections for further evaluation.
- Completes all required enrollment activities and paperwork for eligible families/children.
- Implements the research-based Home Visiting Model, including but not limited to child screenings, family assessments, developmental and health monitoring, curriculum implementation, connections to community resources, parent/child connection activities, minimum visit frequency for each family, and participation in monthly reflection supervision with EC Administrator.
- Provides case management for all families on caseload. Case management includes but is not limited to development and ongoing monitoring of Individual Family Goal Plans, transition plans, community referrals, and completion of timely and accurate case management notes per funder and model requirements.,
- Develops visit plans with parents aligned to the Illinois Early Learning Guidelines and Home Visiting Model that support family well-being, development-centered parenting, parent-child interactions, and early literacy.
- Develops parent and parent/child group lesson plans related to the identified eight (8) designated instructional areas (child growth and development, childbirth and childcare, child safety and injury prevention, family structure, prenatal and postnatal care, prevention of child abuse, interpersonal and family relationships, and parenting skill development)
- Plans and facilitates parent education workshops, group connections, and family engagement activities per funder/model requirements and family interest/need.
- Provides families with information, education, referrals, and connections to community resources.
- Provides follow-up and collaborates with community agencies as needed.
QUALIFICATIONS: Bachelor’s degree in early childhood, child development, psychology, social work, family services, or related field required. Additional training in child/family screenings, parenting curriculums, and community knowledge preferred.

WORK CALENDAR: 12 Months
WORKING CONDITIONS: Moderate in-district travel as well as intermittent in-state and out-of-state travel. Intermittent prolonged and irregular hours of work.
