A Substitute School Nurse shall perform the following: 
• provide specialized and specific health care pursuant to student’s individual health care plan or individualized education program as determined by a physician’s or advanced practice nurse’s care orders
• administer medication as required 
• complete and maintain documentation of first-aid and administration of medication as required
• fill out appropriate accident reports 
• maintain effective communication with medical, nursing, and district personnel
• provide nursing care consistent with school nurse certificate at offsite locations and in the school building
• perform other assigned tasks as required by administration.
Substitute nurses that are applying for a County Substitute Certificate need the following:
1. County Application Form (Completed at the Board Office)
2. AppliTrack Application (Completed on-line)
3. Copy of a current and signed NJ Professional Nurse License (Brought to the Board Office)
4.  Fingerprinting - Apply on line at www.nj.gov/education/educators/crimhist 
     a. For new applicants, refer to directions on www.state.nj.us.education/educators/crimhist/new
     b. For archive application requests, go to site in #4 above.  Click on "File Authorization & Make an Electronic Payment".  Choose "Archive Request".  Enter your social security number and fill out the application.  Make your payment and print the confirmation page.  In 2-4 weeks print your approval notice by going onto the above website. 
5.  I-9 Form (Completed at Board Office and bring a passport or a driver's license and social security card)
6  Oath of Allegiance (Completed and notarized at Board Office)
7. W-4 (Completed at Board Office)
8.  Mantoux (TB) test to be taken by a doctor or drug store clinic and read two days later showing negative results.
9.  Bank check or money order in the amount of $125 made out to the "Commissioner of Education".  Substittute certificate is good for 5 years.

Substitute teachers/teacher aides perform the following:
• follow the lesson plan left by the teacher for whom he/she is substituting in accordance with district's philosophy, goals and objectives 
• meet and instruct assigned classes in the locations and at the times designated  
• establish and maintain order in the classroom
• maintain a classroom environment conducive to effective learning 
• take all necessary and reasonable precautions to protect students, materials, equipment and facilities 
• uphold and enforce school rules
• cooperate with school personnel 
• establish and maintain cooperative relationships with other employees
• report to the principal or his/her designee at the beginning and end of the school day
Please note that applicants holding a valid teaching certificate do not require a Substitute Certificate. 
1. Complete the Criminal History Background Check: https://www.nj.gov/education/crimhist/check/
 
• Pay administrative fee(s) for the criminal history background clearance 
• Print out the IdentoGO New Jersey Universal Fingerprint Form from the NJ Department of Education (NJDOE) Office of Student Protection webpage 
• Go to the Identogo Website to schedule an appointment and pay fingerprinting FEE.

• Attend the scheduled appointment time and get fingerprinted.
• You must bring the following  item to your fingerprinting appointment: Photo ID (Foreign passports will no longer be accepted as proof of identification)
 
2. Apply Online for your Substitute Certificate at https://njedcert.force.com/manage/s/login

Please note:
• Once you receive your criminal history report, you will need to upload this to your NJEdCert Portal
• You will also need to request that your university/college or clearinghouse email your transcripts to certapplication@doe.nj.gov.  The State of New Jersey requires 30 college credits to substitute. 
 
3. You will need documentation of a negative Mantoux test.  Your doctor or pharmacy can provide this.

• Bring the Following Items with you to your Appointment with Stacey Kinum
 
1. Social Security Card
2. Valid NJ Driver’s License
3. Valid Passport, if you have one
Finally, after the paperwork and certification process is complete, you will be interviewed, trained, board-approved, and ready to start subbing!
