JOB DESCRIPTION:
 
• Demonstrates leadership and support of fellow personnel in implementation of overall school health program
• Establishes and implements an improvement process for delivery of school health program
• Promotes positive pupil-teacher-nurse-parent-community relationship
• Maintains professional work habits
• Maintains and upgrades his/her professional skills
• Assists with sweep screening as available
• Completes evaluations as requested by the School Assistance Team (SAT)
• Attends Eligibility Committee (EC) meetings and Individual Education Plan Committee (IEP) meetings relative to health issues
• Develops and implements the Health Care Plans for Special Education students identified as having a chronic health condition
• Writes the health concerns component of the IEP
• Acts as liaison between school, physician and other health care providers
• Organizes and provides for the instruction of (with assistance from other qualified instructors) American Red Cross Standard First Aid and CPR
• Keeps records on Standard First Aid and CPR training for personnel
• Requests, purchases and stocks supplies as appropriate for students with special needs
 
 
PERFORMANCE CRITERIA FOR NURSES (Based on State Board Policy 5310):
• Functions as contributing member of school staff
• Exhibits knowledge of school law/policy
• Exhibits knowledge of total school program
• Acts as a liaison between schools and the medical community
• Establishes and maintains adequate health records, such as but not limited to HCP
• Conducts student/parent/teacher conferences when necessary
• Coordinates, refers and follow-up as necessary
• Collaborates with school staff concerning psychosocial and behavioral needs of students
• Periodically reviews health status of students and shares with appropriate staff
• Collects and reports data for School Nurse Needs Assessment/Survey
• Manages student health records on WVEIS (Immunizations, Electron Health Data Care Plans, etc.)
• Assists in assessment and placement of students with exceptional needs
• Assists parent/student in using appropriate community resources
• Assesses signs/symptoms of communicable disease
• Implements and enforces communicable disease policies in collaboration with public health department
• Contacts parent/teacher regarding communicable disease protocols
• Assesses the severity of injuries and performs CPR and First Aid procedures as indicated
• Assesses the severity of illness and acts appropriately
• Coordinates and/or conducts training of school personnel
• Serves as health education resource person

QUALIFICATIONS:

SALARY:  Commensurate with degree level and experience (Contingent upon the availability of funding provided through the Communities In Schools grant)

ESSENTIAL DUTIES AND RESPONSIBILITIES:
The CIS Lead Coordinator of Student and Support Services is responsible for the overall implementation and management of the CIS Model, maintaining the bridge between the school and community and providing leadership for the collaborative process and development of a continuum of services for children, families, and community members.  In broad terms, responsibilities include:
1. Development and implementation of a strategic plan consistent with guidelines and policies established by Calhoun County’s Advisory Team;
2. Recruiting, training, and coordinating the efforts of any and all volunteers;
3. Coordinating activities of individuals and organizations willing to offer programs and services at the school to meet the needs of students, families, and community members;
4. Development and implementation of fiscal controls and procedures, preparation and development of the budget and coordination of bookkeeping and financial record keeping required for grant maintenance, internal reporting and audit compliance, including maintaining needed communication and interaction regarding fiscal matters with the school, BOE, WVDE, other funding agencies and the Advisory Team;
5. Writing grants, developing and enhancing public relations, and promoting the purposes and endeavors of CIS both within and without the district and area.
 
Examples of tasks those responsibilities cover include:
Leadership
• Participate and help coordinate the Advisory Team (to include representatives of the school, appropriate partners, and parents)
• Insure that the Advisory Team meets regularly to be certain that identified needs are being addressed
• Organize and oversee a community resource and needs assessment process
• Ensure monthly tracking is completed
• Seek input from teachers, school staff, LSIC members, parents, and students to determine ongoing needs of students and families
• Participate in grant-seeking and other fund development activities.
• Make regular reports of progress to the Advisory Team, Attendance Director and Assistant Superintendent
• Serve on the School Leadership Team 
Implementation
• Identify and recruit people and organizations willing to offer programs and services for students and families at the school or to assist with operations based on needs
• Serve as key contact for supports, communication and coordination of activities
• Work with the Advisory Team to determine hours of operation, security etc.
• Develop, maintain, and publicize a schedule of activities and activities offered Coordinate and monitor programs and activities
• Implement and maintain a process that encourages referrals to programs and services offered at the school; provides timely feedback to those who make referrals; and alerts the Advisory Team to the need for programs not already offered
• Coordinate cross-referral of students and families between service providers
• Coordinate communication between service providers, teachers, parents and students
• Evaluate and make recommendations of activities to the Advisory Team
• At the administrator’s request, oversee janitorial and security staff responsible for ensuring that facilities are clean and safe for participants
• Prepare a brochure and newsletter made available to students, families and community members.  Submit news articles to the schools’ newsletters and local monthly publications.  Maintain social media outreach.
• Maintain a portfolio of events, participation, highlights, etc. (Including but not limited to  public relations outreach, agendas, sing-in sheets, news articles, photographs/videos)
• Monitor attendance and initiate interventions and support plans as necessary and per policy.
• Provide monthly attendance reports to administration (school and county level)
• Provide monthly student services data to administration (school and county level)
• Oversee and coordinate multi-tier system of support (academic, attendance, behavior and social emotional)
• Implement strategies and oversight of Student Services Team
• County CIS contact responsibilities in conjunction with Assistant Superintendent
• Monitor chronic absenteeism
• Co-chair Attendance TEAM and CIS County Contact
• Attend MDIT, MDT and truency divesion meetings
• Maintain a database of services, referrals, T1, T2, T3 supports
SUPERVISORY RELATIONSHIP:
The Coordinator is an employee of Calhoun County BOE providing services to district schools and is directly supervised by  building administration and Assistant Superintendent. The school’s principal will provide the Advisory Team with a general evaluation of the Coordinator’s performance after the first three months of service and then annually, at the end of the calendar year. The Coordinator will continue to serve the school based on performance evaluations and may discontinue a Coordinator’s employment based on the school administration’s evaluation.
 
TRAVEL:
Travel is expected as necessary to implement the action plans of the CIS including county, regional, state, and national travel.
