Purpose Statement
 
The job of Alternative Program Counselor/Transition Coordinator - is done for the purpose/s of providing support to the instructional process with specific responsibility for managing the schedules and placement of assigned students; appropriately addressing emotional/behavioral/social concerns of children; using problem-solving strategies to provide necessary support; participating in multi-disciplinary teams; providing course curriculum, information and recommendations to students and parents; and assisting in the development of goals and plans for student learning. The position requires the counselor/transition coordinator to work with academic, behavior, and emotionally challenged students and organize transition programs for students while ensuring compliance of indicators 13 and 14.
 
This job reports to the assigned Principal.
 
Essential Functions
• Advocates for children and families (e.g. social agency contacts/ local education agencies/classroom instructors, etc.) for the purpose of providing family and student resources and developing individualized plans that improve options for success.
• Collaborates with school personnel and local education agencies for the purpose of mobilizing needed resources for improved student behavior and student learning.
• Conducts student/parent/staff interviews and surveys for the purpose of determining potential barriers to successful learning in public schools.
• Completes transition assessments and develops transition plans for students as appropriate.
• Works with various entities such as the DD waiver, Social Security Administration, and DVR to connect families to resources related to student IEP goals.
• Conducts eligibility processes (e.g., IEP designations, assessment of student data, etc.) for the purpose of providing accurate designation of students' needs requiring direct counseling.
• Consults with teachers, special education personnel, parents, and administrators (e.g. a variety of special services, etc.) for the purpose of communicating information, gaining feedback, and ensuring the provision of special services.
• Counsels students, parents, and guardians (e.g. 'at risk', orientation, academic and aptitude testing, peer relationships, etc.) for the purpose of enhancing student learning and student success in school.
• Maintains a wide variety of materials (e.g. quantity reports, student behavior, home visit assessments, activities, correspondence, behavior audits, etc.) for the purpose of documenting activities, and providing written reference, as required by law, school district policy, and administrative regulation.
• Monitors students' progress for the purpose of identifying issues and taking appropriate action to increase student learning and student success.
• Participates in a variety of committees and teams (e.g. all necessary and relevant MDT, IEP Team, case management meetings, etc.) for the purpose of responding to requests of case managers/diagnostician/school psychologist/special education director.
• Provides counseling activities (e.g. counseling/coaching activities, as prescribed by IEP, etc.) for the purpose of ensuring completion of IEP recommendations, and supplemental and necessary student counseling designed to improve student learning.
• Creates and implements transition planning for students moving into the CATS program and then back to regular classroom activities in school.
• Reviews student records (e.g. with instructors and administration, etc.) for the purpose of identifying trends and practices that effectively assist students in achieving learning goals.
• Serves as supplemental provider of direct counseling (e.g. counseling/coaching activities, etc.) for the purpose of providing direction, support, and assistance to designated students.
• Works collaboratively (e.g. local education agencies and community agencies, etc.) for the purpose of mobilizing needed resources to meet needs of students and families.

Essential Functions
• Adapts classroom work for the purpose of providing students with instructional materials that address individualized learning plans within established lesson plans.
• Administers under supervision, first aid, and assistance to medically fragile children (e.g., tube feeding, toileting, diapering, etc.) for the purpose of providing appropriate care for children as assigned.
• Administers developmental testing programs, subject-specific assessments, etc., for the purpose of assessing student competency levels and/or developing individual learning plans.
• Advises parents and/or legal guardians of student progress for the purpose of communicating expectations and student's achievements, developing methods for improvement, and/or reinforcing classroom goals in the home environment.
• Assesses student progress towards objectives, expectations, and/or goals (e.g., behavioral, motor development and communication skills, academic needs, vocational abilities, etc.) for the purpose of providing feedback to students, parents, and administration.
• Collaborates with instructional staff, other school personnel, parents and a variety of community resources for the purpose of improving the overall quality of student outcomes, achieving established classroom objectives in support of the school improvement plan.
• Counsels students for the purpose of improving performance, health status, problem-solving techniques, and a variety of personal issues.
• Creates individualized educational plans (IEPs) for specific students for the purpose of following legal and federal requirements to ensure proper instruction.
• Demonstrates methods required to perform classroom and/or subject-specific assignments for the purpose of providing an effective program that addresses individual student requirements.
• Directs assistant teachers, student teachers, instructional assistants, volunteers, and/or student workers for the purpose of providing an effective classroom program and addressing the needs of individual students.
• Instructs students for the purpose of improving their success in academic, interpersonal, and daily living skills through a defined course of study.
• Manages student behavior for the purpose of providing a safe and optimal learning environment.
• Monitors students in a variety of educational environments (e.g. classroom, playground, field trips, nap times, etc.) for the purpose of providing a safe and positive learning environment.
• Participates in a variety of meetings for the purpose of conveying and/or gathering information required to perform functions.
• Prepares a variety of written materials (e.g. grades, attendance, anecdotal records, IEPs, etc.) for the purpose of documenting student progress and meeting mandated requirements including IEP progress reports.
• Reports incidents (e.g., fights, suspected child abuse, suspected substance abuse, etc.) for the purpose of maintaining the personal safety of students, providing a positive learning environment, and adhering to school policies.
• Responds to emergency situations for the purpose of resolving immediate safety concerns and/or directing to appropriate personnel for resolution.
Alternative Program SPED Teacher Schedule
Alternative Program SPED Teacher will complete ½ day within the alternative program and the other ½ day will be assigned related duties as assigned for the purpose of ensuring the efficient and effective functioning of the district.
